The National Health Facility Survey (NHFS), 2023 is a follow-up to the 2016 and 2019 surveys. The survey modified the Service Availability and Readiness Assessment (SARA), a standard health facility assessment methodology promoted by the World Health Organisation (WHO). The SARA is a systematic health facility assessment tool designed to assist countries in assessing health systems progress, monitoring service availability and readiness to provide services at health facilities. Additionally, it incorporated Service Delivery Indicators (SDIs) survey instruments developed by the World Bank in collaboration with the African Economic Research Consortium (AERC). The SDIs aim to measure primary healthcare service delivery to improve the monitoring of service delivery to increase accountability, good governance, and targeted intervention by assessing the quality and performance of health services from the citizen’s perspective. These tools have been adapted to meet the objectives for consistently monitoring the quality of care for the Federal Ministry of Health and Social Welfare (FMOH&SW) and to provide information needed to strengthen the health system and specific health services at state and national levels. Therefore, the 2023 NHFS covered the availability of basic inputs such as infrastructure and equipment, provider effort, and provider knowledge which are key variables for assessing service delivery at health facilities.

The survey assessed 3,330 primary, secondary and private health facilities selected from the 36 States and Federal Capital Territory (FCT). Ninety (90) facilities comprising primary health centres, secondary facilities and private facilities were randomly selected from each State and FCT. The exercise administered questionnaires covering different areas of service provision. The target respondents include the Officer-in-Charge (OIC) of the facility, Health care providers and caregivers.

The Health Facility Register (HFR) served as the sampling frame for the National and other health facility surveys. Conducting the health facility mapping/listing exercise was a critical step for having a reliable sampling frame for 2023 NHFS, in light of the limitations on the quality of the facility listing such as duplication of facilities, geo-coordinates capturing, misclassification of type of facility and level of care in the HFR.
The listing exercise was carried out in one-third (260) of the total LGAs (774) in the country due to limitation of funding and time constraint. The approach employed was a physical listing of all the health facilities in the selected LGAs. The comprehensive facility listing generated from the exercise informed the randomly selected 3,330 HF sampled in all the 36 States and FCT.
The 2023 NHFS visited a total sample size of 3,330 HFs, a nominal sample of 90 health facilities (HFs) in the 36 States and FCT. Its equal sample allocation stemmed from the intention of delivering estimates of comparable quality for all States, regardless of their population size.
• In the first stage selection, a random sample of 260 Local Government Areas (LGAs) was selected, using a sample frame of the list of all 774 LGAs in the country.
• In the second stage, a random sample of HFs were selected from the sampled LGAs, using an updated list of all facilities.
